Job Description
Provides overall leadership of the supportive housing program, which is designed to support highly vulnerable homeless families in order to engage and ensure appropriate placement in more stable housing. Additionally, provides assessments and comprehensive case management to program participants as a part of a broader housing team. The services are delivered in community settings and support the team in their work. Ensures that files and documentation for participants are complete and updated in accordance with regulatory guidelines. Additionally, completes all documentation for outcome evaluation studies.
Job Duties
- Works with treatment team in completing comprehensive assessments on families enrolled in the program
- Maintain case files for all residents according to regulatory guidelines
- Provide documentation for each service and submits to licensed clinicians for review and signature
- Provides clinical direction and consultation to family case manager
- Work with therapists and team members in the development of monthly reports to track the progress of participants
- Provide crisis intervention to program participants
- Completes outcomes evaluation reports and ensures all clients are entered into HMIS system
- Provide additional assistance to team members as assigned
Requirements
• At least 3-5 years management experience
• At least 3-5 years working in Homeless Services
• Deep understanding and commitment to the issues facing homeless families
• Deep understanding of traumatic stress and working with homeless programs
• Demonstrate sound organization skills including computer literacy
• Demonstrate solid verbal and written communication skills
